There are several forms you may need from time to time during your child’s pre-school education. Hopefully, you’ll find just what you’re looking for here.

These forms are mostly in PDF format and can easily be downloaded and printed out at home. Some we have put up in Word format as well, where they are available, but these files are bigger.

To open and print the PDF forms you will need a program called Adobe Acrobat Reader. Chances are that you will already have this program on your computer. If not, you can download it for free from here.

Beside each form is a brief description of its purpose, so you can be sure you download the correct one.

	Prospectus (PDF - 1.7MB) - This is our preschool prospectus. It’s the biggest download you’ll find on the site. All the information in the prospectus can be found on this website.
	Enrolment form (PDF - 314KB) - This is our enrolment form, which can be opened with Adobe Acrobat Reader.
	20 Hours ECE Attestation Form (PDF - 485KB) Use this form to change the number of hours your child receives under the 20 Hours ECE scheme.
	Direct Debit Form (PDF - 105KB) - Use this form to set up a Direct Debit payment
	Tax Credit claim form (PDF - 104KB) - Use this form to claim a Tax Credit (formerly called a Tax Rebate) from the Inland Revenue.
	Help with Childcare Costs (PDF - 250KB) - A Work and Income guide for parents and caregivers.
	Help when you're Working (PDF - 300KB) - A Work and Income guide for those who are working, or starting work.
	Application form (PDF - 220KB) - Use this form to apply for a Childcare and OSCAR Subsidy
